We are located on the corner of Dutchtown Road and Mohawk Street. We are next to St. Joseph’s Hospital, on the emergency room side. We are directly across from the Boy Scouts of America. We are the tan building with the green roof.

What to Expect
Upon your arrival to our clinic, we will ask for your picture ID and insurance card to ensure we have the most up to date information for you. New patients will be asked to complete registration on the provided computers if pre-registration was not already submitted. Please remember many of our clinics have multiple providers offering different services. We do not limit the number of concerns to be addressed at each visit, therefore, sometimes we may run behind. Please know when it is your turn in the exam room, you will receive the same undivided attention, and we will address all of your concerns. We look forward to seeing you at your scheduled appointment.
